Services at the Laguna Community Health Center (LCHC) will be ramping up beginning March 1, 2021.
Patients who currently receive services at ACL Service Unit and would like to transfer your Medical Records to LCHC, will require a Release of Information Form filled out and taken to ACL. At that point you may either hand carry the records or have them sent to LCHC.
You may also choose to not have any records transferred and start with a new Medical Record at LCHC. At this time Medical Records will not be shared between LCHC and ACL electronically.
LCHC is currently offering the community a preregistration process, which includes the Authorization to Release Healthcare Information and the New Patient Registration Form (see below). The Authorization to Release Healthcare Information should be presented to your current provider, so your medical records can be forwarded to LCHC to create a medical chart to be built for your first visit or appointment. These records will provide demographic information and historical health data that will be verified with your first visit. LCHC will be hosting Drive Through/Drop Off Registration events starting Tuesday, February 16, 2021 from 1 p.m. to 3 p.m. DAILY, at the K-Center Football Field South Parking lot (weather permitting). Additional forms for your family can be picked up at this location at the allotted time. As LCHC staff continues to prepare our facility for opening during the pandemic, no forms can be dropped off at the facility. You may also download the forms below. LCHC will not be accepting any forms via email or electronically as this violates HIPAA and Privacy Act standards. Patient registration staff will be inputting the data and where possible, using the Health Information Exchange (HIE) to download the authorized Protected Health Information (PHI) directly into the LCHC electronic health records. It is the Laguna Healthcare Corporation’s (LHC) Board of Directors, Management team and the Laguna Community Health Center (LCHC) Medical staff’s desire to put forth our best first impression in a COVID safe manner. Because of this intention our Board’s opinion that the initial proposed opening date of February 1, 2021 would not have allowed adequate time to provide essential training for staff, nor would it have allowed LCHC to properly prepare our facility and staff with all necessary COVID precautions and procedures. It is our wish to provide a safe opening for our community to utilize the brand-new facility to its utmost capabilities throughout the pandemic. Though we understand that this extension has caused some inconvenience for some of our partners and stakeholders, the LHC Board continues to feel that it is necessary to be prepared for the safe opening of the new facility for our community.
During this COVID-19 Worldwide Pandemic, we have all come to learn to adjust to the unexpected and have patience with any changes that come our way. The same has been true for many businesses throughout the world, country and more closely, the Pueblo. Unfortunately, this does include the largely anticipated LCHC. Even through the preparation and diligence of LHC and all stakeholders involved in the project, LHC has had to request a contractual 30-day extension to the opening of the Laguna Community Health Center from the Health and Human Services Department through the Indian Health Service (I.H.S.).
The request for the 30-day extension was based on LHC Board, Management and Pueblo of Laguna (POL) Leadership’s consultation to provide the highest COVID safe practices within the facility, and minor delays in facility preparation. Because some of LHC’s vendors dealt with the effects of the surging virus directly through mandatory quarantines both during the construction and installation phases, including the closure of factories impacting equipment deliveries and installation, this caused much of the delay at the end of the project. Furthermore, as a heightened safety measure for the Pueblo, COVID Safety plans are required of all our vendors, who are subject to COVID Screenings and COVID Safety protocols to enter the Pueblo boundaries, which all our contractors and vendors have diligently abided by.
The LCHC Medical Staff, which consists of two Board Certified Family Medicine Physicians, two Nurse Practitioners and other medical professionals, have completed paperwork for their background checks, onboarding, and training, as well as credentialing for all medical professionals. LHC requested that the Intergovernmental Personnel Act (IPA) staff from ACL be afforded the opportunity to begin training on February 1 with LCHC staff prior to opening for continuity of care, unfortunately that was not allowed due to the extension request. The final terms of IPAs are still under negotiations and impact eight (8) positions which will not be released by ACL for reassignment until LCHC’s new opening day of March 2. Those eight positions include Nursing staff, PRC personnel, Custodial/Maintenance, Medical and Triage Assistants. LHC has also recently added to our organizational chart, the position of Physical Therapist which has also been filled by an IPA.
In addition to Medical Services, other services at the new facility will include Pharmacy, Lab, X-ray, a Minor Procedures room (non-surgical), Pediatric rooms, Telemedicine with both internal and external options, integrated Behavioral Health services and Physical Therapy. There will not be an emergency care capability but there will be urgent or non-appointment services available during operational hours. Beginning hours of operations are Tuesday through Friday, 10:00am to 6:30pm and Saturday, 10:00am to 3:00pm. LCHC will be closed on Sunday and Monday and observe Monday holidays on the prior Saturday. This provides greatest coverage of available hours of service to our community. As the patient load grows, so will the hours of operation and staffing.
Please be aware that regardless of the opening date of the LCHC, services at the local I.H.S. facility are still available regardless of tribal membership. ACL will continue to provide services to all beneficiaries as required by law during this extension period and beyond. Neither the Pueblo of Laguna nor LHC have been consulted as to any services being offered at ACL as reference in the Dear ACL Patient letter dated January 5, 2021 from the acting ACL CEO. We are making every attempt to partner with the facility to continue other services through the extension. If you have driven by on New Mexico 124 you should see the beautiful architecture that is becoming more evident with each day of construction. General Contractor Bradbury Stamm is making amazing progress on the construction of the new Laguna Community Health Center, and is on schedule for completion for Winter 2020. Laguna Healthcare Corporation is preparing to begin negotiations with the Indian Health Service for shares to operate the facility and we have begun hiring for several management positions in the facility.

The Laguna Community Health Center construction has begun at the site near the K-Center area. LHC will update as often as we can with the progress. As of July 2020, steel work is underway and the skeleton of the structure is being constructed. Construction is currently on schedule to be completed in December of 2020, with an opening scheduled for Early Spring 2021.
